RESIDENTIAL CARE SERVICES
The Residential Care Hamlets will provide the best in professional support and medical intervention, while maintaining an optimum lifestyle for residents who require 24-hour nursing care. Each resident will have a individualized care plan that recognizes his or her unique interests, history and family situation. As families and informal caregivers often provide care at home until an individual reaches the advanced stages of their illness, we will actively support the links between our residents and their families and friends. The Hamlet layout supports not only small groupings of residents but is designed to promote a family and community style of living. Extended family members are always welcome and they are encouraged to choose their own level of involvement.
The Hamlets accommodation model lends itself specifically to the opportunities of providing specific programming or care services to unique groups of individuals. Examples of such programming are for young adult residents, respite care, end of life care, acquired brain injury and convalescent care.
We understand that individuals living at The Evergreen Hamlets at Fleetwood can come with or develop complex physical and psychological needs that we best address through interdisciplinary teamwork. To this end, evidence based best practices are utilized and our staff actively seek learning opportunities. Our philosophy ensures that the best possible care and services are provided with dignity and respect to each resident.
The Hamlets has both publicly funded and private pay beds, and works with various funding partners, including the Interior Health Authority, Community Living BC, Ministry of Housing and Social Development, WorkSafeBC, ICBC and Private Insurers to meet the varied needs of persons. An interest in a funded bed would begin with a referral from your local representative of the funding body. Private pay beds can be accessed directly by contacting the site.
